Severe Acute Diffuse Otitis Externa

Description : Severe pain and itching are the commonest symptoms and the ear is tender to the touch. If the diffuse swelling of the ear canal is extensive, there may be occlusion of the meatus with resultant hearing loss. Weeping from the excoriated, edematous skin surface may produce a few drops of thin serous or seropurulent discharge. On examination, the lining of the ear canal is edematous and swollen and the lumen is diminished in caliber. The absence of cerumen is characteristic. The external canal skin in this patient is diffusely swollen, and a clear watery discharge can be seen within the lumen. The organism cultured was Pseudomonas aeruginosa.
