The pathology of acute otitis externa is that of an acute dermatitis. The dermis and subepithelial layers are edematous and vasodilated. There are acute inflammatory cells in the deep layers of the epidermis and the epidermis itself may be thickened and parakeratotic. Gram's stain of a histologic section may reveal bacteria in the more superficial layers of the epidermis. The epithelial layer is thickened and covered by a thickened layer of keratin squames. The dermis is infiltrated with both acute and chronic inflammatory cells. H&E 145x. |
